- Collaboration between Gramaze caregivers, coordinators, and relevant healthcare professionals
- Adjustments to care plans as needs evolve, with communication and transparency
Consent applies to all agreed services, including:
- Elder Care
- Palliative Care
- Hospice & End-of-Life Care
- Rehabilitation & Physiotherapy
- Mental & Emotional Wellness
- Approved add-on services (e.g. Family Care Training)
4. Non-Emergency Care Acknowledgement
You acknowledge that:
- Gramaze does not provide emergency medical services
- In emergencies, local emergency services or hospitals must be contacted immediately
- Gramaze caregivers are not emergency responders
5. Medical & Care Limitations
You understand and accept that:
- Outcomes cannot be guaranteed
- Care is provided based on professional judgment and available information
- Home-based care carries inherent risks
- Some conditions may require hospital-based or specialist intervention
Gramaze will always act in the best interests of the patient, within the agreed scope of care.
6. Consent to Personal & Health Information Use
By consenting to care, you also consent to:
- The collection, use, and storage of personal and health information
- Sharing relevant information with caregivers and healthcare professionals strictly for care delivery
- Secure communication of updates with authorised family members
All data is handled in accordance with the Gramaze Privacy Policy.
7. Family & Representative Involvement
Where family members or representatives are involved:
- They may receive updates, reports, and care summaries
- They are expected to communicate honestly and promptly
- They should collaborate respectfully with caregivers and coordinators
Families may also participate in care through approved training programmes.
8. Right to Ask Questions & Withdraw Consent
You have the right to:
- Ask questions at any time about care, risks, or procedures
- Request clarification before or during care delivery
- Withdraw consent, subject to reasonable notice and contractual terms
Withdrawal of consent may affect the ability to continue services.
9. Safety & Environment Responsibility
Families agree to:
- Provide a reasonably safe home environment for caregivers and patients
- Inform Gramaze of any risks, hazards, or changes in condition
- Respect caregiver boundaries and professional standards
Gramaze reserves the right to suspend care if safety is compromised.
10. Confirmation of Understanding
By proceeding with Gramaze services, you confirm that:

- You have had the opportunity to ask questions
- You voluntarily consent to care under the terms described
